The 25-year-old Irishman hit thirty-eight goals in 149 appearances over three seasons for County.
He began his career at Cheltenham Town as a trainee, was loaned to Bath City before moving to Kidderminster Harriers then joining Newport in February 2009.
He has represented his country, Republic of Ireland, at under-18 level.
The npower League One side`s manager Gary Johnson said "Sam won't be a familiar name to Yeovil supporters but he has had two great seasons of success with Newport in the non-League. We beat several other League clubs to his signature and I am very hopeful Sam will keep on improving.”
